Percentage of patients aged 18 years and older with a diagnosis of primary open-angle glaucoma (POAG) who have an optic nerve head evaluation during one or more office visits within 12 months
Numerator
Patients who have an optic nerve head evaluation during one or more office visits within 12 months. Any of the following will qualify:
- Text in Chart: In the Assessment or Plan of the patient's chart, both of the following phrases should be included:
- optic disc exam for structural abnormalities performed
- study of cup to disk ratio performed
- Manually Imported Item: The test name for the lab results/imported item should include the phrase "Optic disc"
Denominator
All patients aged 18 years and older with a diagnosis of primary open-angle glaucoma. Patients must meet all the following criteria:
- Age: 18 years old or older
- Encounter: The patient has an encounter within the measurement period with the appropriate CPT code.
- Diagnosis of Primary open-angle glaucoma: The patient has an appropriate Diagnosis code assigned to the problem list.
Denominator Exceptions
Documentation of medical reason(s) for not performing an optic nerve head evaluation:
- Text in Chart: Free text in plan or assessment any of the following phrases:
- Optic disc exam not tolerated
- Optic disc exam contraindicated
- Optic surgical complications
- Optic disc exam complications
