CMS149v13: Dementia: Cognitive Assessment

Percentage of patients, regardless of age, with a diagnosis of dementia for whom an assessment of cognition is performed and the results reviewed at least once within a 12 month period

Numerator

Patients for whom an assessment of cognition is performed and the results reviewed at least once within a 12 month period

  • Text in chart: In the encounter in the Assessment or Plan fields, or in the Cognitive and Functional Status section of the Summary Sheet, if any of the following terms are found, the patient will be added to the numerator: 
    • 6CIT
    • 6Mini Cog
    • BIMS
    • SLUMS
    • Qcode
    • AD8
    • MMSE
    • MoCA
    • BOMC
    • MDSv3
    • MDSv4

Denominators

All patients, regardless of age, will be added to the denominator if they meet both of the following criteria:

  • The patient has two or more encounters with the physician, identified by an appropriate CPT code.
  • They have a diagnosis of dementia with an appropriate ICD-10 code assigned to the patients problem list, added before the routine cognitive assessment.

Denominator Exceptions

Documentation of patient reason(s) for not assessing cognition

  • Free text in Plan or Assessment any of the following phrases:
    • Refuses cognitive assessment
    • cognitive assessment refused
    • Dissatisfied with result of cognitive assessment
