Numerator
Women with at least one chlamydia test during the measurement period. This can be recorded in any of the following ways:
- An electronically received test's name contains the phrase “Chlamydia”
- Text in Chart: The Plan or Assessment text box contains the phrase “Chlamydia test done”
- An imported item's name containing the phrase: “Chlamydia"
- Any of the following Clinical Decision Support rules completed
- Screening for Chlamydial Infection in Sexually Active Women Aged 24 Years and Younger
- Screening for Chlamydial Infection in Women with Increased Risk
Denominator
Women 16 to 24 years of age who are sexually active and who had a visit in the measurement period
- In the social history, assessment, or plan field, free text the phrase “sexually active”
Denominator Exclusions:
Women who are only eligible for the initial population due to a pregnancy test during the measurement period and who had an x-ray or an order for a specified medication on the date of the pregnancy test or six days after the pregnancy test.
Pregnancy tests can be recorded in any of the following ways:
- An electronically received test's name containing any of the following phrases:
- Pregnancy
- Beta hCG
- The Plan or Assessment text box contains the phrase “Pregnancy test done”
- An imported item's name containing the phrase: “Pregnancy” or “Beta hCG”
- Any of the following Clinical Decision Support rules completed with “pregnancy test done” in the comments:
- Screening for Chlamydial Infection in Sexually Active Women Aged 24 Years and Younger
- Screening for Chlamydial Infection in Women with Increased Risk
- Isotretinoin prescribed within 7 days of any of the above instances
- Any radiology result imported within 7 days of the pregnancy test. Results must be imported to the “Radiology” folder in Imported items.
Exclude patients who were in hospice care during the measurement year.
- Identified in the patient's Alerts and Directives, located in the demographic screen or the Summary Sheet, in the name, text or comments saved with that alert/directive. The Alert/Directive should include either of the following phrases:
- palliative
- hospice
