The percentage of adolescent patients 12 to 17 years of age and adult patients 18 years of age or older with major depression or dysthymia who reached remission 12 months (+/- 60 days) after an index event.
Numerator
Patients who achieved remission at twelve months as demonstrated by the most recent twelve month (+/- 60 days grace period) PHQ-9 score of less than five.
- These 12 months start at the time of the index date (aka the initial encounter with the PHQ-9 > 9)
- PHQ-9 Score: They have a PHQ-9 score of less than 5 within the 12 months (plus 60 days) after the initial visit.
- PHQ-9 scores should be saved in Tracked Data to count for this measure. The name of the tracked data item should include "PHQ-9" or "PHQ9".
Denominator
Adolescent patients 12 to 17 years of age and adult patients 18 years of age and older with a diagnosis of major depression or dysthymia and an initial PHQ-9 or PHQ-9M score greater than nine during the index event. Patients may be screened using PHQ-9 and PHQ-9M up to 7 days prior to the office visit (including the day of the office visit).
Patient’s age 12 to 17 years of age and adults 18 and up will be added to the denominator if they meet all of the following criteria:
- Diagnosis: They have a diagnosis of MDD or dysthymia:
- Encounter: They have a patient encounter during the measurement period with an appropriate CPT code.
- PHQ-9 Score: They have a PHQ-9 score of greater than 9. PHQ-9. Scores should be saved in Tracked Data to count for this measure. The name of the tracked data item should include "PHQ-9" or "PHQ9".
Denominator Exclusion
A patient who meets the criteria to be in the denominator will be excluded from the denominator if they meet any of the following criteria.
- Patients who died any time prior to the end of the measure assessment period: marked in the Demographics as Inactive with a reason of "Deceased"
- Exclude patients whose hospice or palliative care overlaps the measurement period:
- Identified in the patient's Alerts and Directives, located in the demographic screen or the Summary Sheet, in the name, text or comments saved with that alert/directive. The Alert/Directive should include either of the following phrases:
- palliative
- hospice
- Identified in the patient's Alerts and Directives, located in the demographic screen or the Summary Sheet, in the name, text or comments saved with that alert/directive. The Alert/Directive should include either of the following phrases:
- Patients with a diagnosis of bipolar disorder anytime prior to the end of the measure assessment period
- Patients with a diagnosis of personality disorder anytime prior to the end of the measure assessment period
- Patients with a diagnosis of schizophrenia or psychotic disorder anytime prior to the end of the measure assessment period
- Patients with a diagnosis of pervasive developmental disorder anytime prior to the end of the measure assessment period
