Percentage of the following patients - all considered at high risk of cardiovascular events - who were prescribed or were on statin therapy during the measurement period:
All patients who were previously diagnosed with or currently have an active diagnosis of clinical atherosclerotic cardiovascular disease (ASCVD); including an ASCVD procedure
OR
Patient's aged 20 to 75 years who have ever had a low-density lipoprotein cholesterol (LDL-C) level >= 190 mg/dL or were previously diagnosed with or currently have an active diagnosis of familial hypercholesterolemia;
OR
Patients aged 40 to 75 years with a diagnosis of diabetes
OR
Patients aged 40 to 75 with a 10-year ASCVD Risk score of >= 20 percent
Numerator
Patients who are actively using or who receive an order (prescription) for statin therapy at any point during the measurement period
- The medication needs to be either prescribed through the script writer, or entered into the patients "Current Medication" list. The provider does not need to be the prescribing provider for this measure to count.
Denominator
All patients who meet one or more of the following criteria (considered at "high risk" for cardiovascular events, under ACC/AHA guidelines):
- Patients with clinical ASCVD diagnosis (i25.9)
- Diagnosis added to the patients problem list
- An electronically received test name contains the phrase LDL-C and value greater than or equals to 190
- The LDL-C result is saved in Tracked data and the name of the tracked data item includes "LDL-C" and the value is greater than or equals to 190
- Patients aged 20 to 75 years at the beginning of the measurement period who have ever had a direct laboratory result of LDL-C >=190 mg/dL or were previously diagnosed with or currently have an active diagnosis of familial or pure hypercholesterolemia
- Patient currently has an active diagnosis of familial hypercholesterolemia
- Patients aged 40 to 75 years at the beginning of the measurement period with a diagnosis of Type 1 or Type 2 diabetes
- Patients aged 40 to 75 with a 10-year ASCVD Risk score of >= 20 percent
- The ASCVD Risk Score result is saved in Tracked data and the name of the tracked data item includes "ASCVD Risk Score"
Denominator Exclusions:
- Patients who are breastfeeding
- In the physical exam, plan, or assessment field, enter the phrase “breastfeeding”
- Patients who have a diagnosis of rhabdomyolysis
Denominator Exceptions
- Patients with adverse effect, allergy, or intolerance to statin medication
- Exclude patients whose hospice or palliative care overlaps the measurement period:
- Identified in the patient's Alerts and Directives, located in the demographic screen or the Summary Sheet, in the name, text or comments saved with that alert/directive. The Alert/Directive should include either of the following phrases:
- palliative
- hospice

- Patients with a diagnosis of active liver disease, hepatic disease, or insufficiency
- Patients with end-stage renal disease (ESRD)
