Numerator
Eligible clinician attests to documenting, updating or reviewing the patient's current medications using all immediate resources available on the date of the encounter.
When a patient is seen at ANY visit, the clinician will need to do the following ON THE DATE OF THE ENCOUNTER to add the patient to the numerator.
- Open the Current Medications screen for the patient,
- check the Medication Reconciliation Complete checkbox in the bottom right
- click Save Changes and exit to return to the encounter.
Denominator
All visits occurring during the 12 month measurement period.
Denominator Exception
Patient is in an urgent or emergent medical situation where time is of the essence and to delay treatment would jeopardize the patient's health status
In the Plan or Assessment field, add any of the following phrases:
- Urgent Situation. Medication not updated
- Emergent Situation. Medication not updated
- Medical contraindication indicated. Medication not updated
