Percentage of children, ages 1 year to 20 years of age, who received 2 fluoride varnish applications during the measurement period as determined by a dentist Note: This measure can only be completed by a dentist
This will be stratified into three groups:
- Entire Population
- Children ages 1-5
- Children ages 6-17
- Children ages 18-20
Numerator
Patient must have TWO application of fluoride during the measurement period on separate visit dates
The patient will appear in the numerator using one of the following methods:
- Clinical Decisions Support Rule: The client needs to complete the following CDS Rule" , "Dental Caries: Screening/ fluoride varnish" during the measurement period
- Text in the Encounter: In the Plan or Assessment fields of the patient's encounter, use the following phrase:
- application of fluoride
- application of dental fluoride
Denominators 1, 2 and 3
There are three separate denominator, stratified by age groups.
- Denominator 1: Children ages 1 year - 5 years will be added to denominator 1 if they had an dental encounter during the measurement period
- Denominator 2: Children ages 6 years - 17 years will be added to denominator 2 if they had an dental encounter during the measurement period
- Denominator 3: Children ages 18 years -20 years will be added to denominator 3 if they had an dental encounter during the measurement period
Denominator Exclusions
Exclude patients whose hospice or palliative care overlaps the measurement period:
- Identified in the patient's Alerts and Directives, located in the demographic screen or the Summary Sheet, in the name, text or comments saved with that alert/directive. The Alert/Directive should include either of the following phrases:
- palliative
- hospice
