When adding a new provider / prescriber for Amazing Charts, the new provider / prescriber will need to prove their identity in order to prescribe within Amazing Charts. This document takes a step-by-step approach to completing this process.
Electronic Prescribing Identity Proofing Instructions

- From the Main Screen of Amazing Charts, select “E-Prescribing” from the top menu. Note you must be logged into the new providers account.
- Select “Identity Proofing” from the drop down
- Enter your information, select “Save”
- Select “Start Identity Proofing
- You will receive an automated email to the email address entered in Step 3.
- Follow the link in the email. More detailed instructions for the ID Proofing process can be found at the below link. Note that if the provider has a credit freeze this will not work. Alternate instructions can be provided if needed.
- You will receive a verification code via postal mail. When the code arrives, enter it into the “verification code” box.
