PHQ-9 Not Saving to Chart

The PHQ-9 form doesn't save directly into the chart on its own. It hands the answers to a set of Amazing Charts services running quietly in the background on your server, and those services write the results to the database.

When one of those services stalls, nothing tells you so. The form simply doesn't save. Restarting the services gets them talking again, and the PHQ-9 saves normally on the next try.

Read this first

Hosted on Amazing Charts Cloud? These steps won't work for you, and there's nothing you need to try. Cloud customers have no direct access to the server, so this has to be fixed on our end. Contact Technical Support using the details at the bottom of this page.

Everyone else: these steps are performed on the server computer, not on a workstation. If you're not sure which machine is your server, or you don't have administrator access to it, stop here and contact Technical Support rather than guessing.

Step by step

Go to the server computer

Sit down at the server itself, or connect to it remotely. The services you're about to restart only exist on that machine, so doing this from a workstation won't help.

Search for Services

Click the Start button (the Windows icon) or the search box next to it, and type services.

Look for the result named Services with a gear icon, listed as an App. That's the one you want.

Open Services

Click the Services result. A window opens listing everything running in the background on that computer. The list is long, and it's alphabetical.

Find the Amazing Charts services

Scroll to the A's. You're looking for every service whose name begins with Amazing. On most servers there are five:

Seeing only four? That's normal on some installations, particularly older ones that don't have the API Helper Service. Restart whichever Amazing services you find. Don't go looking for a missing one.

Restart each one

Right-click the first Amazing service and choose Restart from the menu.

Windows will briefly show a progress box while it stops and starts the service. Wait for that to finish and for the Status column to read Running again before moving to the next one.

Repeat for every Amazing service in the list.

No Restart option? If the menu shows Start in black instead, that service is stopped rather than stalled. Click Start instead, and mention it to support afterwards. A service that stops on its own is worth a look.

Try saving the PHQ-9 again

Go back to the workstation where the problem happened. Open the patient's chart, fill in the PHQ-9, and save it.

Anyone who had Amazing Charts open during the restart may need to close and reopen it before things behave normally.

Re-enter the PHQ-9 from scratch rather than assuming the earlier attempt was kept. If it didn't save, the answers weren't stored.

If it doesn't work

What you're seeing What to try
Still won't save Restart the services once more and try again. If it fails a second time, stop and contact support rather than repeating the restart.
A service won't restart Note the exact wording of the error Windows gives you and contact support. A service that refuses to start usually points at something needing a direct look.
A service restarts, then stops Same as above. Don't keep restarting it. Make a note of the service name and call support.
Only one user is affected Have that user close and reopen Amazing Charts first. If it's still only them, the problem is on their workstation, not the server.
Other forms won't save either That points at something broader than the PHQ-9. Contact support and describe which forms are affected.
It keeps coming back Services stalling repeatedly is a symptom, not the problem. Contact support so the cause can be found.
